Polished Deepslate is naturally generated in Ancient Cities. If you can find an Ancient City, you can gather Polished Deepslate with a pickaxe.- If you use any other tool, the Polished Deepslate will drop nothing.[1]

Place 1 Cobbled Deepslate in the lower-left corner of a 3x3 crafting grid, then place 1 additional Cobbled Deepslate directly above, diagonal, and to the right of the initial Cobbled Deepslate.- You will get 4 Polished Deepslate when crafting it.
